As antikodon noted, this is 3C 215, which is a WAT (wide-angle tail) radio source, with a QSO as host (zsp 0.411 SDSS J090631.87+164611.8). It's a single radio source, though I must say, however, that the radio morphology does look strange, even for a WAT.
There's a separate, faint, #compact radio source to the SWS, with zsp 0.438 host SDSS J090631.04+164522.6:
